Comparison of the efficiency of transperineal vs. endoanal scan in detection of anal sphincter injury - Ireland

Anal sphincter injury is following 1% of all vaginal deliveries. Prospective studies, using Endoanal Ultra Sound (EUS), have identified sonographic abnormalities of the anal sphincter anatomy in up to 36% of women after vaginal delivery. However, the relevance of these findings is unknown, since majority of patients with such ultrasound finding are asymptomatic. The EUS is considered now a gold standard in diagnosis and follow up of patients with anal sphincter injuries. However, there are some limitations associated with its use. While the External Anal Sphincter (EAS) is clearly seen there are some problems with visualisation of the Internal Anal Sphincter (IAS) due to transanal compression. The transperineal 4‐Dimensional Ultrasound Scan (4‐DUS) similarly offers a high detection rate of 3rd and 4th degree perineal tears.
